About WQ HCH

The Western Queensland Health Care Homes (WQ HCH) provide proactive patient-centred, coordinated and flexible care with a team of professionals working together to make sure the patient receives care based on their needs.

A patient who is eligible and likely to benefit from this type of care can sign up to become a WQ HCH patient. Together with the patient, a care team at the WQ HCH then develops a shared care plan.

The shared care plan:

  • Includes comprehensive information about a patient’s care, medications and all the health professionals who care for them
  • Identifies local health care providers best able to meet each patient’s needs (including access to telehealth), and helps coordinate care with these providers
  • Include strategies to help each person manage their conditions and improve their quality of life.

Our WQ HCH is different to the HCH national trial as it has been specifically tailored to suit people living in rural and remote parts of Australia (to find out more click here)

The WQ HCH is being rolled out in stages commencing July 2019. To find out which practices are participating please click here.
