WQ HCH Model of Care is supporting practices to provide the right care, in the right place at the right time by the right team. At the heart of the model is a whole-of-system integration approach that is focused on improving the capacity and capability of general practice.
The model is conceptualised within three core domains of care including: Ready Access to Care; Proactive Preventative Care and; Engaged Management of Chronic and Complex Care.
