What are the Benefits of WQ HCH for Me / My Practice?

Working together in a team-based manner means sharing the load. This not only reduces some of the burden on GPs and nurses, it also allows other members of the health care team to utilise the full extent of their training and maximise their professional capacity. This collaborative, team-based approach to care has been shown to increase job satisfaction and provide a better work-life balance.

Benefits of participating include:

  • Having an experienced dedicated WQPHN Coordinator to:
    – Improve patient engagement and experience
    – Cleanse, manage and assess population and practice data to optimise business performance
    – Develop, and improve, sustainable quality improvement systems and processes
    – Build more engaged teams who are responsive to improving quality and sustainable change
  • Having an opportunity to influence co-design and trial new models of care
  • Having access to professional development activities that build the capacity and capability of the practice team
  • Having access to a Community of Practice (CoP) through a support network of other WQ HCH practices to share learnings and best practice
  • Being informed and assisted in how to respond to current and emerging health policy direction.

How will the practice be supported to implement the WQ HCH?

WQPHN will offer a range of ongoing support (link to Support for Uptake for more information), in line with practice needs and will work in partnership to provide support for uptake of the WQ HCH Foundations. The Practice Coordinators will:

  • Work directly with the practice team
  • Help identify areas for improvement based on the WQ HCH Maturity Matrix (to assess the transformation journey)
  • Support to implement patient satisfaction tools
  • Use QI programs and training to improve systems and processes
  • Provide training support in care planning, recall and reminder systems, Practice communication methods, POPGUNS training, motivational interviewing, CAT4Plus, health literacy and cultural awareness training.
  • Assist with the development of Practice registers
  • Use of WQPHN Service Frameworks to help with priority setting and planning
  • Support PIP After Hours registration and uptake
  • Support PIP incentive plan and uptake
  • Support to register for Q Viewer
  • Support uptake and meaningful use of My Health Record
  • Provide support to interpret the Practice Performance Analysis (PPA)
  • Support the development of a WQ HCH Implementation Plan
  • Support to implement WQPHN digital health strategy
  • Provide practice and population data reports
  • Provide education, resources and communication tools
  • Support for connecting care with the WQ HCH neighbourhood
  • Support with tracking and monitoring progress
  • Support with Accreditation
