About WQ HCH

Nearly one in five people living in Western Queensland have at least two chronic health conditions. These people need services from different health professionals working in different locations including specialists who are mostly based in cities or larger regional centres. Often there is a lack of coordination and communication between the different parts of the health system, which causes frustration for patients, their families and carers.

The Western Queensland Health Care Homes (WQ HCH) provide proactive patient-centred, coordinated and flexible care with a team of professionals working together to make sure the patient receives care based on their needs.

Our WQ HCH is different to the HCH national trial as it has been specifically tailored to suit people living in rural and remote parts of Australia (to find out more click here).

The WQ HCH is being rolled out in stages commencing in July 2019. If you think you would benefit from being a WQ HCH patient (if you are assessed as being eligible) and would like to find out if your practice is a participating please click here.
